    Do orthodontists account for facial aesthetics in their treatment plans?

    I have what is called bimaxillary protrusion whereby both my upper and lower teeth are protruding. All three orthodontists I have seen said braces with 4 extraction + surgery would be the best option. I opted to go ahead and get invisalign with 4 premolar extraction, but without surgery.

    My ortho just sent me the clincheck he approved. What excites and scares me is how much my front teeth will move backwards at the end of the treatment. On the one hand, it means that my protrusion problem will improve significantly. On the other hand, it means my facial profile is likely to change dramatically.

    My question is: does the ortho automatically account for facial aesthetics and will come out with a treatment plan that will aim to change my facial profile for the better? I want to correct my teeth only for aesthetics reason- I have no problem with my bite and suffer no lip strain. I read horror cases online that some faces "collapse" after extraction.

    I am very worried because extractions are simply irreversible.

    I don't know but I have the same worries like you. After Invisalign my both jaws are supposed to come in about 1-2 mm (I have one missing tooth and since I also have a bit crowding I was thinking to move the crowded teeth to fill the space from the missing tooth as much as possible). I have talked to my dentist about it and according to her this will not change my face because it is too little. But I am not sure, because now, when I have my trays on although they are very thin I can see difference in my face and other people also ask me what has happend with my lips. So if this is obvious the opposite change also should be obvious. I was considering to change my treatment but since my dentist says that it will be OK and since I have not seen the result I can not be 100% sure that I want to change the treatment. Så I have decided to continue and see how is going and take decision later. In general I think that the goot orthodonts consider that face proportions when they straighten teeth. But deffinitely NOT ALL. You must explain to dem clear that you don't want big profile changes and since you are the one who pays they must account it
