Retainers are needed to keep your teeth where they have been moved to. Your teeth will continue to move while you're alive, so to keep them where your orthodontic treatment has moved them to, you will need a commitment to retainer wear for life. For most people this begins with a period of full time wear, then every night and for most it's then down to a couple of nights a week for life.
The 'bits and pieces' that are used during jaw surgery usually do stay in your jaw, but they don't have to. Some people have these removed once the bone has grown and secured the areas it needs to.
Side affects and other things ... ask your ortho and surgeon if they can tell you about any.
